Booking: Telephone Consultation with Provider

Duration: 15 minutes

Location: Telephone

Fee for Service: $30 (15 minutes)

Limitations: Telephone consultations that exceed the scheduled duration are subject to rescheduling for the remainder of the consultation.

Payment: We are an "out of network" provider. All fees must be paid in full at the time of your session. As an "out of network" provider, we will give you a receipt that you may submit to your insurance company for reimbursement. Please check with your insurance to see if your benefits cover services from an "out of network" provider. Thank you. Add on services are not likely to be reimbursed by your insurance provider as an "out of network" claim.  

This service is available for urgent issues and questions as well as for your convenience 

if a problem must be addressed prior to your next appointment. 

This service is not intended to replace any in-office appointments for follow-up treatment.
